Assisted Living Transition

When it comes to assisted living, finding the correct facility is only half the battle. The transition period is a crucial time, and our staff understands that seniors can experience fear, socialization issues, loss of independence, and other difficulties in their adjustment. When a senior needs someone to speak up for them or help with overcoming challenges, our Certified Senior Advisors are there to go above and beyond as a friend, ally, and advocate during these transitions.
Assisted Living Transition will include but not limited to:
- Ensuring all nursing care is being met and meds are being handled properly.
- Checking that meals are prepared and handled properly, and residents are visiting the dining hall.
- Encourage socialization with other residents.
- Activity participation.
- Make sure the client is overall satisfied with their experience.
- Check in with the leadership team to see how the resident is handling the transition.
- Comfort and companionship.
- Emotional support.
- 1-2 hour visits.
Follow up services can be included for an added fee.
A Certified Senior Advisor will document via "Next Stage Visit Log" detailing the overall visit. The document will be emailed to a family member(s) to provide an update.
A complimentary phone call to check in at night before bed to make sure day went well and phone calls logged via "Next Stage Phone Call Log" and emailed to a family member(s).
